چکیده
Case A patient with juvenile onset recurrent febrile attacks associated with familial polyarteritis nodosa (PAN) and who carries CECR1 and MEFVgene mutations is described. The index case, a 23 year old male patient with recurrent attacks of fever and arthritis since the age of 7 was diagnosed initially as Familial Mediterranean Fever (FMF). A beneficial response to treatment with colchicine was observed. A year later he developed livedo reticularis and nodular dermal lesions compatible with cutaneous PAN. He was treated with prednisolone and azathioprine. Arthralgia, fever and dermal lesions regressed and he was in remission until he developed anemia and macrocytosis a year later and azathioprine was stopped. Due to the activation of his skin vasculitis anakinra 100mg/day was instituted. The beneficial response obtained with anakinra was lost when he discontinued the treatment. A family history revealed a brother two years older than himself who also had livedo reticularis, Raynaud’s phenomena, fever and arthritis since the age of 8, diagnosed as PAN before the index case and died at age 22 because of gut perforation secondary to acute mesenteric ischemia. With the probable diagnosis of ADA2, the index patient was analyzed for CECR1 gene mutations on chromosome 22q11.1. After amplification of the exons 2, 4, 5, 6 and 9 on PCR, DNA sequencing analysis was performed. A homozygous c.139G-A transition in exon 2, resulting in a gly47-to-arg (G47R) substitution at a highly conserved residue in the dimerization domain was identified in this patient who was known to be heterozygous for M694V and R202Q mutations of the MEFV gene. His vasculitic lesions responded to Infliximab after the fourth infusion together with daily colchicine treatment.

MEFV Gene Profile in Northwest of Iran, Twelve Common MEFV Gene Mutations Analysis in 216 Patients with Familial Mediterranean Fever
Familial Mediterranean Fever (FMF) is a hereditary autoinflammatory disease with autosomal recessive inheritance pattern often seen around the Mediterranean Sea. It is characterized by recurrent episodes of fever and polyserositis and rash. Recently, MEFV gene analysis determines the definitive diagnosis of FMF. In this study, we analyzed 12 MEFV gene mutations in more than 200 FMF patients, pr...

Background & objective: FMF is an autosomal recessive genetic disorder characterized by acute episodes of fever accompanied by severe abdominal pain, pleurisy, arthritis, and skin rashes. Clinical symptoms of the disease vary considerably in patients, and little is known about the origin of this variation. This study investigates a male patient suffering from severe FMF and his parents. Materia...

Background and objectives: Behcet's Disease (BD) is a rare severe recurrent inflammatory disorder affecting several body organs. Since Familial Mediterranean Fever (FMF) and BD affect almost a specific population, both diseases can mimic the other clinically, and these two diseases sometimes occur in the same family and the same patient, also due to the high prevalence of BD in Iran and perform...
متن کاملMEFV mutations in Northwest of Iran: a cross sectional study
Objective(s):Familial Mediterranean Fever (FMF) is an autosomal recessive disorder characterized by recurrent episodes of fever accompanied by peritonitis, pleurisy, and arthritis. FMF affects mainly Mediterranean populations and is caused by mutations in the familial Mediterranean fever (MEFV) gene. The aim of this study was to identify the frequency and distribution of MEFV mutations in Irani...

Background : Familial Mediterranean fever (FMF) is an autosomal recessive disease, which is characterized by recurrent short episodes of inflammation in serous membranes. It is most prevalent in Western Mediterranean population. MEFV is the only gene currently known to be associated with this disease. Previous studies revealed that 6% of Iranian Jewish residents in Israel were carriers of MEFV ...
